Whose a "Provider"?
1. Qualified Medical Providers (QMP) and QMP proxies (staff designated by the QMP to help manage patients)
2. Limited Medical Providers (LMP). Providers who can recommend medical cannabis for patients without registering through DHHS
3. Pharmacy Medical Providers (PMP). Pharmacists that are also physicians that can write prescriptions

Qualified Medical Provider (QMP)
Utah-licensed physicians (MD, DO), advanced practice registered nurses (APRN), physician assistants (PA), and podiatrists (DPM) who are licensed to prescribe a controlled substance may receive approval from DHHS to recommend medical cannabis treatment for their patients. These providers must register as a "qualified medical provider" through the Electronic Verification System (EVS).
Limited Medical Providers (LMP)
The Electronic Verification System (EVS) is the Center for Medical Cannabis database used to manage maedical cannabis applications for patients and providers. All patients and QMPs must be registered in the EVS to participate in the program
Pharmacy Medical Providers (PMP)
Pharmacists licensed under opens in a new tabTitle 58, Chapter 17b, Pharmacy Practice Act and physicians who have the authority to write a prescription and are licensed under opens in a new tabTitle 58, Chapter 67, Utah Medical Practice Act, or opens in a new tabTitle 58, Chapter 68, Utah Osteopathic Medical Practice Act may become "pharmacy medical providers" (PMP). PMPs must register through the Electronic Verification System (EVS).
Please note that applicants must have been offered employment at a medical cannabis pharmacy in order to be issued a pharmacy medical provider card. Do not apply until you have been offered employment. Any fees paid will not be refunded if an applicant applies prior to being hired by a licensed medical cannabis pharmacy.
PMP applicants must:
- Complete four hours of DHHS-approved continuing education and submit certification of completion with registration application. Find out about approved CE courses by visiting our Continuing Education page.

Meetings & Minutes
The Center for Medical Cannabis hosts a quarterly meeting for qualified medical providers (QMPs), QMP proxies, pharmacy medical providers, and limited medical providers.
These virtual meetings will take place on the second Thursday of the quarter at 8 a.m. We’ll answer your questions and discuss topics like program updates and statistics, law changes, compliance updates, and resources for recommending medical cannabis to patients.
