About us
The Compassionate Use Board (CUB) includes 7 medical providers from different specialties. Our role is to:
- Review medical cannabis card petitions for patients who don’t have a qualifying condition but would still benefit from medical cannabis and patients younger than 21 years. The patient’s medical provider submits petitions for us to review.
- Coordinate with the Utah Department of Health and Human Services (DHHS) to issue waivers for medical cannabis patients to purchase more cannabis than the state limit each month for patients on palliative care. Purchase waivers are for patients who have a terminal illness, are expected to live for 6 months or less, and who need medical cannabis for palliative care.

How do I submit a petition to the Compassionate Use Board (CUB)?
The patient’s medical provider submits petitions for us to review. The process is different if you are a recommending medical provider (RMP) that is registered in the electronic verification system (EVS) or if you are an RMP that sends a certification directly to a medical cannabis pharmacy.
Submit a petition in the EVS
- Meet with the patient in-person after they complete their medical cannabis card application in the EVS. Use the medical cannabis patient assessment tool to help decide if medical cannabis is a good treatment option for them.
- Enter the medical cannabis certification in the patient’s EVS application.
- Create a CUB petition in the patient’s EVS application. The EVS will prompt you to do this.
- Fill out the petition and submit it in the EVS.
- The patient will log into EVS and make their payment. Their EVS application status will change to “Awaiting CUB Review.”
- Add [email protected] to your email contacts. We’ll send updates about the petition from this email address.
- You’ll get an email from DHHS with our decision within 90 days of when the patient pays the application fee.
Send a certification form to a pharmacy
- Meet with the patient in-person after they complete their medical cannabis card application in the EVS. Use the medical cannabis patient assessment tool to help decide if medical cannabis is a good treatment option for them.
- Fill out the recommending medical provider recommendation form for medical cannabis and send it to a medical cannabis pharmacy.
- The medical cannabis pharmacist will enter your certification in the patient’s EVS application.
- The patient will log into EVS and make their payment. The EVS status will change to “Awaiting CUB Review.”
- Add [email protected] to your email contacts. We’ll send you an email with a link to the Compassionate Use Board petition through a REDCap survey. You’ll also get updates about the petition from this email address.
- Fill out the petition and submit it through REDCap.
- You’ll get an email from DHHS with our decision within 90 days of submitting the petition.

1 - What information should I include in a Compassionate Use Board petition?
We use the information in the petition to weigh the patient’s risks and benefits of using cannabis. We look at several factors, including the patient’s age, medical history, and treatments they’ve tried or are currently using.

2 - What if you need more information about the petition?
We’ll email you and your patient from [email protected] if we need more information about the petition.
The email will include the date that we need the information back by. We’ll inactivate the petition if we don’t hear back from you in time.
3 - Can I attend the Compassionate Use Board meeting about my patient’s petition?
Compassionate Use Board meetings are private and confidential, even to the patient’s medical provider. Only board members and specific DHHS staff can attend these meetings. We will email you and your patient updates about the petition.
How do I request a patient purchase limit waiver?
- Meet with the patient in-person after they complete their medical cannabis card application in the EVS.
- Enter the medical cannabis certification in the patient’s EVS application.
- Email [email protected] and ask for a purchase limit waiver application. We’ll send you a link to a REDCap survey.
- Fill out the REDcap survey and submit it. The application will ask for the following information:
- The patient’s medical diagnosis
- If the medical condition is terminal
- If the patient needs medical cannabis for palliative care.
- A chart note from the patient’s most recent palliative care visit.
- You’ll get an email from DHHS with a decision within 10 days of submitting the application.
