Follow these steps to apply for a patient card:
- Create a Utah ID account if you don’t already have one. Follow the steps at https://dts.utah.gov/idhelp/account-creation/ to create your account.
- Create an account in the electronic verification system (EVS). You can use the patient EVS guide if you need help. This guide has more detail and pictures of each step in the EVS.
- Submit a medical cannabis application in the EVS. You can use the patient EVS guide if you need help.
- Meet in-person with a medical provider. Your provider will submit a certification through the EVS that verifies that you meet the qualifications to use medical cannabis. Find a medical provider near you.
- You’ll get an email with instructions to pay your $15 medical cannabis card fee after your provider submits a certification. You’ll get an email stating that your card is active after you pay the fee.
- If you’re younger than 21 years or you’re using medical cannabis for a medical condition that’s not in the law, you’ll need to wait for your application to be reviewed by the Compassionate Use Board before your card becomes active. Your medical provider will submit your application to the Compassionate Use Board and you’ll get an email with a decision after their review. Most of the time, you’ll hear back within a month, but it can take up to 90 days.
- Once your medical cannabis card is active, you can download or print it from your EVS account. You must show your card and a valid ID every time you go to a medical cannabis pharmacy. Find a medical cannabis pharmacy near you.
- You will meet with a medical cannabis pharmacist the first time you go to a medical cannabis pharmacy. Call the pharmacy to see if you need to schedule an appointment with the pharmacist.
- Your card will last for 1 year unless your medical provider marks a shorter renewal period on your certification.
Renewing your card is very similar to applying for your first card. You will use your same UtahID and EVS account.
You’ll receive an email about renewing your card 30 days before your card expires. You can renew your card before it expires, but you won’t get a new card until your current card expires.
Follow these steps to renew your card:
- Go to evs.utah.gov and click the “Log in” button.
- You will be taken to the UtahID log in page. Log in with your UtahID.
- On your electronic verification system (EVS) account page, click the “Patient” button on the left hand side.
- Click the “My application” button under the “My profile actions” heading.
- Click on the application you need to renew under “Patient ID”. Your application status will change to “Active pending renewal” 30 days before your card expires.
- Click the "Resubmit application" button on the left hand side.
- Check your contact information and make sure it’s correct. Update any new information. Click the “Next” button on the right side of the screen.
- Read the patient acknowledgment statement and click the “Acknowledge“ box.
- Click the “Submit for certification” button.
- Your medical provider must enter a new certification for you after you’ve resubmitted your application. Contact your medical provider and set up an appointment to renew your medical cannabis certification.
- You’ll get an email from us with instructions to pay your medical cannabis card fee after your provider submits your new medical cannabis certification in the EVS.
If your status is 'Renewal Completed' you are finished. You will continue to use your current card until its expiration date. The date after your current card expires you will receive your new card. If your current card is expired you will receive your new card the day after you have paid.

A medical cannabis guardian is the parent or legal guardian of a provisional patient (a medical cannabis patient who’s younger than 18). Guardians buy medical cannabis for their child and manage their medical cannabis card.
Follow these steps to apply for a guardian and provisional patient card:
- Create a Utah ID account if you don’t already have one. Follow the steps at https://dts.utah.gov/idhelp/account-creation/ to create your account.
- Create an account in the electronic verification system (EVS). You can use the guardian/provisional patient guide if you need help. This guide has more detail and pictures of each step in the EVS.
- Submit a medical cannabis application in the EVS for your child. You can use the guardian/provisional patient guide if you need help
- Take your child to meet with their medical provider in-person. The provider will submit the medical cannabis certification and a Compassionate Use Board (CUB) petition in the EVS. You’ll get an email from us with instructions to pay your medical cannabis card fee after these are submitted. The CUB won’t review your petition until you pay the application fee. - After you pay the application fee, you’ll get an email from [email protected] with information on how to complete your fingerprint background check. Your cards won’t become active until you complete the fingerprint background check, even if the CUB has already approved your petition.
- If your child’s CUB petition is approved, you can download or print your guardian medical cannabis card and your child’s provisional patient card from your EVS account. You must show your card and a valid ID every time you go to a medical cannabis pharmacy. Find a medical cannabis pharmacy near you.
Medical cannabis card fees:
- Guardian and provisional patient cards (initial): $67
- Guardian and provisional patient cards (renewal): $24
- Secondary guardian and provisional patient cards (initial): $15
- Secondary guardian and provisional patient cards (renewal): $15
A medical cannabis caregiver is someone who can buy medical cannabis at a medical cannabis pharmacy for patients who can’t go themselves.
A medical cannabis patient must request you as a caregiver in their Electronic Verification System (EVS) account. The EVS is an online database that holds all of the medical cannabis cards, recommendations, and purchase data for patients and medical providers.
Follow these steps to get a medical cannabis caregiver card:
- You’ll get an email from the EVS after the patient requests you as their caregiver.
- If you don’t see a request in the EVS, make sure the patient entered your birthday and email correctly. Email us at [email protected] if your information was entered wrong.
- Create a UtahID account if you don’t already have one. Follow the steps at https://dts.utah.gov/idhelp/account-creation/ to create your account.
- Register as a medical cannabis caregiver in the EVS. You do not need to create a new EVS account if you already have one.
- Complete the caregiver request in the EVS by submitting an application. You can use the caregiver EVS guide if you need help. This guide has more detail and pictures of each step in the EVS.
- You will need to pay a registration fee and complete a background check before you get your caregiver card and buy medical cannabis for your patient. You will get emails from us with instructions for both of these tasks.
Medical cannabis card fees:

- Caregiver card (primary patient initial): $67
- Caregiver card (secondary patient initial): $15
- Caregiver card (first renewal): No charge
- Caregiver card (primary patient subsequent renewals): $14

- To complete a fingerprint background check, you’ll need to print and fill out a “live scan fingerprinting authorization form” found in the EVS and bring it to a Livescan vendor. You’ll pay for your fingerprint background check and the Livescan vendor will scan your fingerprints for the background check.
- Note: If you’re a caregiver for a patient with a terminal illness, your card will be active when you pay the card fee. You then have 60 days to complete the fingerprint background check. Your caregiver card will be inactivated if you don’t complete the fingerprint background check within 60 days.
- We’ll review your application once we get the results of your fingerprint background check. This can take up to 15 days, but normally is approved within a day or 2. You’ll get an email from us when your medical cannabis card is active.
- Once your medical cannabis card is active, you can download or print it from your EVS account. You must show your card and a valid ID every time you go to a medical cannabis pharmacy. Find a medical cannabis pharmacy near you.
- The patient and/or caregiver will meet with a medical cannabis pharmacist the first time they go to a medical cannabis pharmacy. Call the pharmacy to see if you need to schedule an appointment with the pharmacist.
If you have a medical cannabis card in your home-state for one of the following medical conditions, you can apply for a Utah non-resident medical cannabis card.
- acute pain
- acquired immune deficiency syndrome (AIDS) or HIV Alzheimer’s disease
- amyotrophic lateral sclerosis
- autism
- cancer
- cachexia
- Crohn’s disease or ulcerative colitis
- epilepsy or debilitating seizures
- multiple sclerosis or persistent and debilitating muscle spasms
- persistent pain that lasts longer than 2 weeks.
- post-traumatic stress disorder (PTSD)
- a terminal illness that with a life expectancy of less than 6 months you’re receiving hospice care for
- a rare condition or disease as defined by the National Institutes of Health.
You can use a non-Utah resident medical cannabis card to buy and use medical cannabis while you visit Utah. These cards last 21 days and are only given out 2 times each year.

Follow these steps to apply for a non-Utah resident medical cannabis card:
- Create a UtahID account if you don’t already have one. Follow the steps at https://dts.utah.gov/idhelp/account-creation/ to create your account.
- Log in to the EVS at evs.utah.gov using your UtahID. You’ll then create an account in the EVS.
- Submit your application in the EVS. You can use the non-Utah resident guide if you need help. This guide has more detail and pictures of each step in the EVS.
- You’ll get an email from us with instructions to pay your $15 medical cannabis card fee. Pay this fee before you travel to Utah, or it may take extra time for your card to be activated.
- Your card will be active on the start date you requested in your medical cannabis card application.
- Once your medical cannabis card is active, you can download or print it from your EVS account. You must show your card and a valid ID every time you go to a medical cannabis pharmacy. Find a medical cannabis pharmacy near you.
- You will meet with a medical cannabis pharmacist the first time you go to a medical cannabis pharmacy. Call the pharmacy to see if you need to schedule an appointment with the pharmacist.
